For most Indian families, Vietnam and Japan should not be compared as two versions of the same MBBS-abroad route. Vietnam's named partner universities offer a direct application pathway built around Class 12 eligibility and a six-year medical programme. Japan has six-year undergraduate medicine too, but the official Study in Japan material and individual university pages show that medical study is commonly Japanese-medium and admission is faculty-specific. The decisive question is whether the student can genuinely study and complete clinical work in the required language.

Admission and academic fit
Vietnam's current partner data gives families a practical shortlist screen. Minimum Class 12 thresholds vary by institution, including subject-specific criteria at Can Tho University of Medicine and Pharmacy and lower stated thresholds at some private universities. Dai Nam is currently the partner exception requiring a qualifying NEET result at admission. For other listed universities, NEET is not presented as an admission requirement, but students still need to understand the India-side pathway for foreign medical graduates.
Japan requires a different first question: does the actual medical faculty admit international students into its undergraduate medicine programme? The official Study in Japan portal says application materials and entrance examinations vary by school. Its examination guidance also notes that some medical and dental programmes may require a common examination system used by Japanese students. Parents should ask whether an Indian Class 12 science qualification is accepted, which examinations apply, whether a preparatory year is needed, and whether the route is medicine rather than biomedical science or public health.
Do not compare “direct admission” with “Japan is academically better” as if either statement answers eligibility. A student who cannot qualify for the specific medical faculty has no usable route, regardless of the country's reputation.
Language is the central decision filter
Vietnam families should request a year-by-year medium-of-instruction statement. It should cover lectures, practicals, examinations, ward rounds, patient histories, records and internship. English classroom teaching does not remove the need to communicate with Vietnamese-speaking patients. Read the site's English-medium and clinical-language guide before accepting a brochure claim.
Japan is more demanding for a student seeking an English-first route. The official Study in Japan FAQ says that students entering Japanese-taught or mixed-language university courses generally need substantial Japanese ability and gives N2 and EJU benchmarks as guidance, not a universal rule. The official school listing for Okayama University's six-year Medicine programme identifies the medium as Japanese. This does not prove that every Japanese medical course is identical, but it shows why a parent must verify the exact faculty.
Ask the Japanese university for the language used in anatomy, practical examinations, patient communication, clinical records and internship. Also ask whether Japanese language preparation is included in the offer, separately charged, or a condition before the medical course begins. A student may manage a Japanese language class and still not be ready for clinical communication.
Fees and the full six-year budget
Vietnam's current partner data provides a useful planning range for named universities, from approximately $4,100 flat annual tuition at Dai Nam to higher scheduled public-university tuition. Hostel, food, insurance, visa, travel, document work and currency changes must be added. Use the site's cost-of-living budget guide to structure questions rather than accepting a generic monthly promise.
Japan's official academic-fees guidance gives broad undergraduate planning figures and notes that actual fees vary by institution. Medicine may carry higher faculty costs than a general undergraduate average. Obtain the medical faculty's tuition per year, admission fee, examination fee, laboratory and clinical charges, insurance, accommodation deposit, language-school cost and likely annual increase. Add the cost of travelling home and the financial requirement for a Japanese student visa.
Compare like with like: six years of tuition plus living costs, not one country's tuition with the other's first-year estimate. Keep original currency quotes and record the exchange-rate date.
Clinical training, local registration and NMC checks
The NMC's rules and regulations page links current FMGL materials. The official FAQ and advisories address conditions including duration, English medium, theory, practical and clinical training, internship and the graduate's registration or eligibility in the country of qualification. These are checks to apply to a named course, not a country-wide approval.
For Vietnam, request the affiliated hospital list, clinical timetable, supervision model, patient-language support, internship document and evidence of local eligibility. For Japan, ask whether the international student can complete the whole clinical course and internship in the same institution, what Japanese proficiency is required, and what local medical registration or eligibility a graduate receives.

Visa, residence and parent support
Vietnam students should follow the university's current immigration workflow and confirm temporary residence after moving into the hostel or rented room. The site's student visa and residence guide lists the documents and renewal questions.
Japan's official Study in Japan guidance and the Japanese Visa Portal should be used with the university's instructions. Ask who applies for the Certificate of Eligibility, how the student obtains the visa, when residence registration occurs, what health-insurance steps follow arrival and how a change of accommodation is reported. Never plan a six-year course on an assumed tourist or short-stay route.
