For most Indian families, Vietnam is the more accessible route to investigate, while South Africa is a highly selective, faculty-specific option with limited international places. South Africa should not be treated as a simple higher-cost alternative to Vietnam: the University of Pretoria says places for non-South African citizens are limited and gives preference to SADC applicants, while its 2026 prospectus says applicants from countries that have medical schools will not be considered except under intergovernmental agreements. Vietnam partner universities may assess Class 12 subjects, marks, passport, health documents, English evidence and an interview or other screening. The University of Pretoria publishes 300 first-year students selected annually, limited international places and preference for SADC applicants. For an Indian Class 12 student, the practical question is not “Which country has easier MBBS admission?” It is “Will the exact faculty accept this nationality and qualification, and can the student meet its selection process?” Get a written answer from admissions rather than relying on an agent's general list.
Fees and family cash flow
Vietnam's local partner data includes different fee models, from published annual figures to year-wise schedules. Treat those as planning context until the institution issues an international invoice. South African universities set their own fees and international charges. Do not use a domestic fee or a general internet estimate for a non-South African applicant. Ask the faculty for the 2026 international fee, payment deadline, annual increase, registration charges, insurance or medical-aid requirements, residence cost and refund conditions. Convert the budget into rupees with a contingency for rand movement and do not assume a six-year total is fixed.
Compare both options on cash flow:

South African medical faculties operate in a professional environment where English is widely used, but the exact course still governs teaching, assessment and patient placement. The student should ask whether international students can access the same clinical sites, what language support is expected, and how registration or internship requirements affect the route.
Vietnam may offer English-medium classroom teaching, yet a student can still meet Vietnamese-speaking patients, nurses and hospital staff. A “100% English” claim without a clinical plan is incomplete.
Language is not a reason to reject either country automatically. It is a reason to understand the student's daily clinical reality before paying a deposit.
Clinical training and professional registration
For Vietnam, request the named hospitals, departments, years of rotation, supervision, attendance rules, patient-facing language, internship structure and completion documents. South Africa has its own regulator, the Health Professions Council of South Africa. HPCSA states that registration is a prerequisite to practising and that foreign qualifications do not guarantee registration; the council may require an evaluation or board examination. This matters if a family imagines South Africa as an easy route to practise there after graduation. Ask the university and HPCSA how the exact qualification, internship and registration categories interact.
If the student plans to return to India, assess both routes under Indian rules. No comparison can promise NMC registration, NExT or any exam result. Course duration, clinical exposure, patient language and internship evidence matter more than a country label.
Visa, residence and family support
For Vietnam, confirm the university's invitation or sponsorship process, entry document, temporary-residence registration, accommodation address and emergency contact. Use the official Vietnam e-visa portal for current immigration information, but follow the university's student-specific instructions where they differ.
For South Africa, confirm the study-visa route, medical or health-cover documents, proof of funds, police-clearance requirements where applicable, registration timing and accommodation. Use the South African university and official government channels rather than a generic checklist. The student should know who handles a missed flight, lost passport, illness or a change of residence.
Which family profile may fit?
Vietnam may merit deeper review when the student needs a realistically accessible foreign-university application route, the family can manage a new clinical language environment, and the family is willing to verify WDOMS, MOET information, hospitals, internship and NMC requirements.
South Africa may merit review only when a named faculty confirms that the Indian applicant is eligible, an international place is genuinely available, and the family understands the fees, competition, visa conditions and HPCSA pathway. A minimum score is not an offer, and an offer is not a licence.
